Make an Impact:
- Provide compassionate, direct patient care to elderly or disabled individuals.
- Provide care in accordance with the plan of care developed by the RN and physician.
- Build friendly, professional rapport and provide reliable support to clients with chronic conditions, such as diabetes.
- Administer medications, injections, wound care, and treatments as prescribed.
- Monitor and report vital signs, symptoms, and patient progress.
- Assist with personal care needs, as appropriate within the nursing scope of practice.
- Maintain accurate, timely, and complete documentation in compliance with state and agency requirements.
- Recognize and report changes in client condition, as appropriate.
- Promote safety and infection control for clients and coworkers.

- Current license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse in the state(s) of employment.
- Graduate of an accredited school of professional nursing.
- Current CPR certification.
- Ability to work independently and manage time effectively.
- Strong verbal and interpersonal communication, observation, and critical-thinking skills.
- Compliance with HIPPA and all privacy and confidentiality policies.
