About the Role:
ComForCare Home Health Care (North San Diego) is looking for a compassionate and dedicated Home Care Nurse, LVNs, LPNs to join our growing team in San Diego, CA. This is a rewarding opportunity to make a real difference in patients' lives by delivering high-quality, personalized care right in the comfort of their own homes. Responsibilities:
- Conduct in-home patient assessments and develop individualized care plans
- Administer medications, treatments, and skilled nursing procedures as prescribed
- Monitor and document patient health status, progress, and changes in condition
- Coordinate with physicians, therapists, and care team members to ensure continuity of care
- Educate patients and family members on health management, medications, and safety
- Provide wound care, IV therapy, and other skilled nursing interventions as needed
- Maintain accurate and timely clinical documentation in compliance with regulations
Requirements:
- Active RN or LVN license in the state of California
- Previous home health, home care, or community nursing experience preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Valid California driver's license and reliable personal transportation
- CPR/BLS certification required
- Compassionate, self-motivated, and comfortable working independently

Compensation: $40.00 - $42.00 per hour
